Permission to use photographs and/or video footage.

During the course we may take photos and videos. St Jude's will use such photograps and/or video footage in good judgment in accordance with your permission.

Please check all the options that apply below:

I consent to recognisable photographs and/or video footage of me being used in the following manner:

displayed in public noticeboards within St Jude's

for St Judes in-house publicity - including printed and projected images (for example to advertise Taste & See)

in printed material for use external to St Judes (for example, in Welcome Packs or publicity brochures)

on the St Jude's church website

in social media (FB & insta)

I do not wish recognisable photographs or video footage of me to be used in any context.
